3D Guided Implants Case: Full Mouth Rehabilitation Journey from USA to Hyderabad

This case study follows the journey of a cardiovascular surgeon who flew from the Western United States to our clinic, Dr Jaydev Dental, for a microscopic root canal procedure. After successfully addressing his root canal issue, he expressed concerns about his upper back teeth, which were supported only by a denture. As a food enthusiast, he wanted a fixed solution that would allow him to chew comfortably for the rest of his life. We utilized 3D digital scans, CBCT, and guided implant placement to restore both functionality and aesthetics in just a few visits.

The Challenge

Mr. Smith, the patient, had six firm teeth in the front of his mouth but had significant issues with his upper back teeth. These were replaced with a denture that no longer provided him with the necessary comfort and function. As a food lover, his main concern was being able to chew food comfortably every day without limitations.After completing his microscopic root canal, Mr. Smith expressed his desire to retain his front teeth and sought a permanent, fixed solution for his upper back teeth. His goal was to chew and eat without restriction for the rest of his life, which meant finding a solution for his upper back teeth that would be reliable, durable, and comfortable.

The Approach

Phase 1: Diagnosis and Treatment Planning

To begin the process, we performed 3D digital scans and CBCT imaging to assess his dental condition, focusing on the upper back teeth and bite alignment. After reviewing the results, we presented multiple treatment options for Mr. Smith’s posterior bite.The initial plan included two implants for the back region, with an additional implant at the backmost region using a sinus lift procedure. However, after discussing his concerns, the patient was strongly opposed to the sinus lift. To accommodate his preferences, we recommended an innovative approach—pterygoid implants, which would allow us to place the implants without the need for the sinus lift.

Phase 2: Implant Placement and Digital Planning

We took 3D digital scans and created a digital guide for pterygoid implant placement. This approach ensured precision in the placement of the implants, minimizing any risks and improving overall success rates. The guided implant placement allowed for optimal positioning, ensuring that the implants were placed in the correct angles for maximum stability and function.The surgical procedure was carried out by our experienced team, including an implantologist, prosthodontist, and critical care specialist. During the procedure, six implants were placed in total—two implants in the front region and one pterygoid implant at the backmost region. The surgery was completed successfully, and the patient was provided with temporary dentures to wear while healing.

Once the implants had integrated into the bone, we moved to the prosthetic phase, in which digital scans were used to create custom Zirconia and ceramic prostheses. 3D planning continued in this phase, ensuring the final prosthetics were not only functional but also aesthetically aligned with the patient’s facial features and personal preferences.

Results

After completing the implant placement, Mr. Smith was thrilled with the immediate improvements. In a short period, he transitioned from using a denture to having a stable, fixed solution. His upper back teeth were restored with pterygoid implants, providing him with both functionality and comfort. The treatment allowed him to chew freely, restoring his confidence in his ability to enjoy food without limitations.
